I think the waitress in the video is part of the set up. The "subjects" are the other customers to see their reactions.
Also, I think it is very telling that the old bastard at the end of the video is too much of a coward to admit that he approves of how the waitress treats the gay customers. It's typical. Also, this would seem to indicate that he knows it is wrong.
You're right about the waitress. This is part of ABC's hidden camera show "What Would You Do?"
